SACJ revised paper templates

These new templates and LaTeX and BiBTeX files at http://sacj.cs.uct.ac.za/index.php/sacj/about/submissions#authorGuidelines conform to the standard introduced in December 2015 for SACJ submissions. The only significant difference from the published layout is that we do not require APA Version 6 referencing in review versions.
